All players at Greenwood Lake Sports Paintball Field are required to fill out the "Waiver Form" form before they are permitted to enter the playing fields. All players under the age of 18 must have the "Waiver Form" signed by a parent or guardian. (Please Note: The minimum age to play paintball is 10 years old.) In addition, all players under the age of 18 need to fill out the "Medical Permission Form" and have it signed by a parent or guardian.

The required forms may be downloaded below. The waiver forms are available in Adobe PDF and Microsoft Word (.DOC) formats. It is recommended that you print and fill out the necessary forms in order to save time during check-in in on your day of play. No person will be admitted on the playing fields unless the proper forms are filled out and signed.
